Permit setup
- Job description, location and equipment boundary defined.
- Responsible person and competent worker identified.
- Affected departments notified.
- Emergency, alarm, radio, bridge, bilge, fire, AV/IT or guest-service impacts identified.
- Work window and permit expiry time recorded.
Isolation
- All energy sources identified, including generators, shore power, emergency generator, UPS, batteries, inverters and stored energy.
- Isolation points locked and tagged.
- Tag numbers recorded.
- Test instrument checked on a known live source.
- Absence of voltage verified at the work point.
Work controls
- Stored energy discharged or controlled.
- Contractor supervised or escorted as required.
- Stop-work authority understood.
- Temporary power and test equipment reviewed.
- No other person removes an authorised lock.
Closeout
- Tools, covers, guards and temporary leads cleared.
- Locks and tags removed by authorised persons only.
- Equipment restored in controlled sequence.
- Alarms and affected systems tested.
- Drawings, defects and closeout notes updated.
