- Date, time and time zone recorded.
- Vessel location and operating mode recorded: underway, alongside, anchor, marina, yard or guest trip.
- Event type selected: full blackout, partial blackout, shore-power trip, generator trip, bus fault, PMS fault or local distribution fault.
- Duty engineer, bridge contact and reporting person recorded.
- Immediate safety concerns recorded: smoke, fire, flooding, electrical smell, abnormal noise or trapped personnel.
Source and switchboard state
- Online generators before the event recorded.
- Running generators after the event recorded.
- Emergency generator or emergency source status recorded.
- Shore-power status, current limit and trip indication recorded where applicable.
- Main switchboard and emergency switchboard indications photographed.
- Breaker positions, busbar voltage and busbar frequency recorded before reset where safe.
Alarm and automation evidence
- Generator controller alarms photographed.
- PMS alarms photographed.
- UPS alarms, battery status and runtime indications recorded.
- Load-shedding events recorded.
- Synchronising, reverse-power, overcurrent, earth-fault or protection alarms recorded where shown.
- Logs exported before normalising where possible.
Essential service status
- Bridge informed and status updates recorded.
- Steering support status checked.
- Navigation and radio support loads checked.
- Bilge, flooding and fire alarm monitoring checked.
- Machinery monitoring and engineers' alarms checked.
- Critical network, SatCom or monitoring UPS loads checked where operationally required.
Recovery sequence
- Recovery decision recorded: automatic recovery, manual generator start, shore-power transfer, load-shed recovery or stop-and-investigate.
- Essential loads restored before hotel and guest comfort loads.
- Generator restart attempts counted and reason recorded.
- Voltage, frequency and load sharing checked after reconnection.
- Hotel loads restored in stages.
- Any bypass, manual mode or temporary workaround recorded.
Closeout and corrective action
- Suspected cause recorded.
- Corrective action owner assigned.
- Evidence pack location recorded.
- Defect, PMS job, class/flag/management notification or incident report reference recorded where applicable.
- Post-event review completed before closing the defect.
- Blackout recovery sequence updated if the event exposed a gap.
